Procedure Description:
The right and left groins were prepped and
draped in the usual sterile fashion. After local infiltration with 2%
Xylocaine, the right femoral artery was entered. 6fr Arterial sheath inserted in the right femoral
artery. AO opening pressures were recorded. The pigtail catheter was advanced
retrograde into the LV. After left heart pressures were recorded, LV-AO
pullback was performed. Selective coronary angiography performed in multiple
projections. Pre-dilatation of RCA performed. Stent to RCA deployed. Sheath
sutured in place for subsequent angioplasty. DSD applied. Examination of
peripheral pulses following procedure reveals adequate perfusion
LV ANGIOGRAM: The RAO projection showed a
Mildly Impaired overall
CORONARY ANGIOGRAPHY:
Overall Dominance: Right
Right
Coronary Artery: A Complex, 99% occlusion was found in the Distal RCA region.
Left Main: The left main artery is angiographically unremarkable.
Left Anterior Descending: There were 2 lesions found in the Left
Anterior Descending. An Eccentric,
<30% occlusion was found in the Prox LAD region. An Eccentric, <30% occlusion was found in the Mid LAD region.
Left Circumflex: A Concentric, 75-80% occlusion was found. in the Prox Cx region.
Right Coronary Artery Left Coronary Artery
POST-INTERVENTIONAL SUMMARY:
Right Coronary Artery: A stent was successfully deployed in the distal section of the RCA. Post-intervention angiography shows this vessel to be normal.
Left
Circumflex: Post-Stent Angiography demonstrates that a successful Stent
procedure reverted this vessel to its normal anatomy.
RCA: Pre intervention Post Stent intervention.
LCA: Pre intervention Post Stent insertion
CONCLUSIONS:
Mild mitral and aortic valves stenosis.

The patient should be medically. Valvular disease should continue to be monitored.
